- More tissue means more blood volume — the heart pushes harder through more miles of vessels
- Sympathetic nervous system overdrive — excess adiposity keeps the “fight or flight” system chronically activated, constricting vessels
- Kidney sodium retention — insulin resistance and fat-derived hormones make the kidneys hold salt and water
- Sleep apnea — nighttime oxygen drops trigger stress-hormone surges that keep pressure high around the clock, a classic cause of “resistant” hypertension
- Vascular inflammation — inflamed vessels stiffen and narrow
The rule of thumb from weight-loss research: systolic blood pressure falls roughly 1 mmHg per kilogram of weight lost. Lose 15 kg (33 lbs) — a typical result on modern GLP-1s — and the arithmetic alone predicts a meaningful reduction, before counting the drugs’ direct vascular effects.

The Evidence: Blood Pressure Results From the Major Trials #
Semaglutide:
- STEP trials: systolic reductions of roughly 3-6 mmHg versus placebo, alongside 15%+ weight loss
- SELECT trial (17,604 participants): sustained blood pressure improvement contributed to the 20% reduction in major cardiovascular events
Tirzepatide:
- SURMOUNT-1 ambulatory substudy (published in Hypertension, 2024): 24-hour ambulatory monitoring — the gold-standard measurement — showed systolic reductions of about 7-8 mmHg at 36 weeks
- Reductions held across daytime and nighttime readings — important because nighttime pressure best predicts cardiovascular risk
Context: every 5 mmHg systolic reduction lowers major cardiovascular event risk by ~10% (meta-analysis, The Lancet, 2021). Tirzepatide’s effect approaches what some single antihypertensive drugs deliver.
An honest caveat: these trials enrolled people with overweight or obesity, not primarily people with hypertension, and GLP-1s haven’t been tested head-to-head against blood pressure medications. They complement antihypertensives — they don’t compete with them.

Who Benefits Most? #
Strongest candidates:
- BMI 27+ with diagnosed hypertension — hypertension is a standard qualifying comorbidity for GLP-1 therapy
- “Resistant” hypertension plus snoring or daytime sleepiness — screen for sleep apnea; treating it may be the missing piece
- People on multiple BP medications whose weight has never been directly treated
- Prehypertension (120-139 systolic) with obesity — addressing weight now can prevent the diagnosis entirely
Important: GLP-1s are additive to blood pressure treatment, not a substitute. Keep taking prescribed antihypertensives, monitor home readings as weight comes off, and let your prescriber adjust doses — some people do get to step down medications over time.
Who should not take GLP-1s: people with a personal or family history of medullary thyroid carcinoma or MEN 2 syndrome, history of pancreatitis (discuss with your provider), and anyone pregnant or planning pregnancy.
